The RN Care Manager uses the nursing process and evidence-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support Technician) and larger Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The RN Care Manager is responsible for collaborating with services internal and external to the VA to meet the patients' needs. This position will float to Martinsville, Shelbyville, Wakeman and Crane Clinics, wherever there is a need for coverage. Duties included but are not limited to: The RN performs, reports, and records assigned nursing care in the Primary Care Clinics, including, but not limited to: Triages walk-ins appropriately Uses appropriate telephone etiquette for both internal and external customers Uses telephone assessment skills to identify actual and/or potential health, safety, and educational needs of the patient Appropriately uses decision support protocols to systematically assess and address caller needs Appropriately prioritizes the urgency of caller needs utilizing critical thinking skills and decision support tools Develops a collaborative plan of care with caller, their support systems, and their provider Completes documentation for clinical calls/TRM's within 24 hours of call Enters as ordered, locates as needed, and obtains results of x-rays, tests, procedures, and labs Incorporates knowledge of ethical/legal issues and professional nursing standards necessary for providing patient care and information Knowledge of and implementation of PACT principles Completes hospital discharge and post discharge ER follow-up calls within two business days Documents hospital discharge and post discharge ER calls in CPRS Utilizes secure message as a means of communication with patients and documents pertinent health information in CPRS Facilitates and acts as provider in RN clinic including providing knowledge/information to patient/family to promote self care of acute/chronic disease management Applies clinical knowledge of primary health care in prevention and wellness, health risk assessment, disease management, and self care Adheres to proper infection control procedures regarding equipment, cleaning, and storage Understands responsibility for the safe processing of reusable medical equipment Follows policies for insuring the correct patient is receiving the correct treatment by using two patient identifiers Performs procedures as direction, i.e. EKG, Doppler, I&O catheterization, venipuncture, PPD application, nebulizer treatments, dressing changes, bladder scan, vital signs and weights, etc. Administers medications as ordered and documents appropriately Adheres to the Missed Opportunities Policy for cancelation of provider/Med RN appointments Follows policies/procedures developed by their service, facility, and VISN Work Schedule: Monday-Friday 7:30a-4:00p Financial Disclosure Report: Not required
