The Nephrology Social Work Coordinator has experience that demonstrates possession of advanced practice skills and judgment within Nephrology and Hemodialysis program. The Nephrology Social Work Coordinator provides services to a patient population with highly complex health and mental health problems. The social worker collaborates with the other members of the Nephrology and hemodialysis Programs in the provision of comprehensive health care services to this special population. The senior social worker will ensure equity of access, service, and benefits to this population and collect, develop and analyze data using appropriate. The incumbent will conduct psychosocial assessments and high-risk screens to determine needs of Veterans and family members. The incumbent is an active participant in the treatment planning process with other disciplines. The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. The incumbent will provide individual and family counseling as appropriate. The incumbent will provide consultation/education to Veterans and families on community resources, advance directives and VA benefits. The incumbent will act as a consultant to other team/staff members. The incumbent will provide consultation/education to medical residents on social work practice and VA policy and procedure. The incumbent will act as liaison between VA and community resources, to include marketing and public relation duties as needed. The incumbent will provide case management to Veterans and families, as needed, throughout the continuum of care. The incumbent will provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. The incumbent will address Advance Directives and Organ Donation in accordance with the standards of their work area. The incumbent will provide patient education services tailored to fit the needs of the Veteran. The incumbent will as appropriate, supervise the practice of social work students, or a less experienced social worker. The incumbent will coordinate treatment and communicate with the care lines to facilitate continuity of care for Veterans. The incumbent will participate in committee assignments, as deemed necessary by the service manager. The incumbent will maintain data and statistical compilations to comply with Joint Commission, VA and medical center policy and procedures. The incumbent will assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. The incumbent will document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. The incumbent will maintain workload productivity within the nationally accepted parameters (minimum 85%-maximum 135%) Other duties and responsibilities may be assigned by the team/group leader as needed for the benefit of the veteran, family, and quality of services provided. The individual works full-time in the Nephrology Program but may be assigned other duties. The incumbent is responsible for completing the following duties/responsibilities: The incumbent will conduct psychosocial assessments to determine needs of veterans and family members. The incumbent makes psychosocial and psychiatric diagnoses within approved clinical privilege and evaluate the veterans' ability to manage services/treatment. Manages and coordinate the Dialysis Contract list (current 122) The incumbent maintains superb interaction with 42 VA Dialysis Unit patients and aid as needed. The incumbent completes renal transplant psychosocial evaluation for patient in Transplant workup. The incumbent reconciles annual reports documenting VA Dialysis Unit Patient population. Complete Medical Reports when patient initiate hemodialysis or peritoneal dialysis. Submit monthly reports to Network 8 documenting changes in patient population. Ensures that Fee for Service Consults (Dialysis Contracts) are now renewed and resubmitted on an annual basis (each fiscal year). Serves as POC for all calls related to dialysis services from numerous clinics throughout Middle Tennessee Area. Assist with arranging Nephrology appointments for patients who have not been seen in the VA system and initiates hemodialysis in the private sector when needed. Point of Contact for various VAMCs transferring patients to the area/program. Coordinate plans and transportation for Veterans leaving the program or traveling to other sites for care. Responds to referrals from other sites. Incumbent participates in the monthly Renal Transplant Committee to screen potential transplant candidates. The incumbent will coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans.
